⚛️ Diagnostic Radionuclides: Current Landscape and Emerging Trends
2026-08-21
Molecular imaging technologies such as PET/CT go beyond anatomical imaging by revealing biological processes at the molecular level. 🔬
Diagnostic radiopharmaceuticals support early diagnosis, disease staging, treatment selection and response assessment. In theranostics, they can also help evaluate target expression, select suitable patients and assess treatment response.
🏥 Two Supply Models
Because many PET radionuclides have short half-lives ⏱️, production location and transportation distance directly influence supply.
Two major models coexist:
🏥 On-site production — Hospitals or molecular imaging centers operate their own cyclotron and radiopharmacy facilities, offering greater flexibility for short-lived radionuclides, routine production and research.
🏭 Centralized production & distribution — Regional radiopharmacies produce radiopharmaceuticals and distribute them to nearby PET centers.
The optimal model depends on population density, clinical demand, transportation conditions and radionuclide requirements. In many markets, the two models will continue to coexist.
🔬 Beyond ¹⁸F: Toward a Multi-Isotope Era
¹⁸F remains one of the most widely used radionuclides in PET imaging, but the field is expanding.
Radionuclides such as ⁶⁸Ga, ⁶⁴Cu and ⁸⁹Zr are gaining increasing attention, while ⁴⁴Sc and ¹²⁴I are opening new research possibilities. 🧪
Compared with conventional ¹⁸F production, emerging radionuclides may require different beam energies, target systems, enriched target materials, and separation and purification processes. Their availability also remains limited in many markets.
As diagnostic radiopharmaceuticals diversify, radionuclide production is gradually moving from a focus on individual isotopes toward flexible, multi-isotope production capabilities. ⚛️
The future of diagnostic nuclear medicine will be defined not only by new tracers, but also by the ability to produce the radionuclides behind them reliably.
